This Consumer Health Data Privacy Policy explains how FAM with Brooke, LLC ("we," "us," or "our") collects, uses, and shares consumer health data, and how you can exercise your rights. It supplements our Website Terms of Use and Privacy Policy and applies to residents of Washington, Nevada, Connecticut, and any other state with similar consumer health data laws.

1. Consumer Health Data We Collect
If you enroll in one of our programs, we may collect:

• Health and cycle-related information you share in your intake form, such as your cycle history, reproductive and pregnancy history, medications or supplements, and health conditions that may affect your charts

• Cycle charts you submit, which may include temperature, cervical fluid, bleeding, and other observations or notes you choose to record

• Health-related questions and messages you send us through our client portal

• Health information you choose to share during live group calls

2. How We Collect It
We collect consumer health data directly from you, through our intake form, chart submissions, client portal messages, and live group calls.

3. Why We Collect It and How We Use It
We collect and use consumer health data only to:

• Provide the program you enrolled in, including teaching you to chart and interpret your cycles

• Review your submitted charts and answer your questions

• Communicate with you about your participation in the program

• Comply with legal obligations

We do not use consumer health data for advertising or marketing, and we do not sell it.

4. Consumer Health Data We Share
We may share the following categories of consumer health data:

• Cycle charts and questions you submit for group calls

• Intake form answers, charts, and portal messages, as stored and processed by our service providers

• Any consumer health data we are legally required to disclose

5. Who We Share It With
We share consumer health data only with these categories of recipients:

• Other participants in live group calls, limited to charts and questions you submit for review, and only with your separate Group Chart Review Consent

• Service providers that process data on our behalf, including our client portal, video call platform, and email provider

• Government authorities or legal parties, when required by law, such as mandated reporting obligations, a subpoena, or a court order

• A successor practitioner, if the Educator becomes incapacitated or dies, as described in our Client Agreement

We do not have any affiliates with whom we share consumer health data.

6. Your Rights
You have the right to:

• Confirm whether we collect, share, or sell your consumer health data

• Access your consumer health data, including a list of the third parties and affiliates we have shared it with and their contact information

• Withdraw your consent to our collection or sharing of your consumer health data, including your Group Chart Review Consent

• Request that we delete your consumer health data

7. Correcting Your Information and Policy Updates
You may ask us to correct consumer health data that is inaccurate by emailing hello@famwithbrooke.com. If we make material changes to this policy, we will update the effective date above and notify enrolled clients by email or through our client portal.

8. How to Exercise Your Rights
To make a request, email us at hello@famwithbrooke.com with the subject line "Consumer Health Data Request." We may need to verify your identity before completing your request. We will respond within 30 days. If we need more time, we will let you know within that period.

9. How to Appeal
If we decline your request, you may appeal by emailing hello@famwithbrooke.com with the subject line "Consumer Health Data Appeal." We will respond to your appeal within 30 days and explain our decision. If your appeal is denied, Washington residents may submit a complaint to the Washington State Attorney General's Office at atg.wa.gov.
